Care Management Support Coordinator II

Location: Corpus Christi, TX

Pay Rate: Starting at $18.00/hour

Schedule: Monday-Friday | 8:00 AM-5:00 PM

Work Arrangement:Remote after training

Dean's Professional Services is seeking a dependable, compassionate, and organized Care Management Support Coordinator II to support a leading healthcare organization. This is an excellent opportunity for a professional who enjoys helping members, coordinating services, and connecting individuals with healthcare and community resources.

This position is remote for candidates in the Corpus Christi area. Training will be conducted at the local Corpus Christi office, after which the position will transition to a work-from-home schedule.

Responsibilities

  • Support administrative care management activities, including member outreach, inbound calls, scheduling, and documentation.

     

  • Contact members by phone to provide support with care plan next steps, health plan and community resources, scheduling questions, and ongoing education.

     

  • Assist members in connecting with health plan and community resources to promote high-quality care and service.

     

  • Serve as a front-line resource for member and provider questions, requests, and concerns.

     

  • Explain care plan procedures, protocols, and available resources as appropriate.

     

  • Support member onboarding and day-to-day administrative activities.

     

  • Prepare and send welcome letters, correspondence, educational materials, and other program information.

     

  • Maintain accurate, non-clinical member records in accordance with state and regulatory requirements.

     

  • Provide appropriate documentation and information to providers as needed.

     

  • Use knowledge of available health plan benefits and local resources to assist members.

     

  • Identify and provide referrals for Social Determinants of Health (SDOH) needs.

     

  • Communicate professionally with members, providers, internal teams, and community resources.

     

  • Maintain confidentiality and comply with all applicable policies, procedures, and regulatory standards.

     

  • Perform other duties as assigned.

     

Qualifications

  • High School Diploma or GED required.

     

  • Previous experience in healthcare, customer service, care coordination, case management support, or an administrative environment preferred.

     

  • Strong communication and interpersonal skills.

     

  • Excellent organizational and time-management abilities.

     

  • Comfortable communicating with members and providers by phone.

     

  • Strong attention to detail and accurate documentation skills.

     

  • Ability to work independently in a remote environment.

     

  • Comfortable learning and navigating multiple computer systems.

     

  • Strong problem-solving skills and the ability to handle member questions professionally.

     

  • Reliable internet connection and appropriate home work environment required for remote work after training.
