Care Management Support Coordinator

Pay Range: $18.5/hour.  Employee benefits include, but are not limited to, health insurance (medical, dental, vision), 401(k) plan, and paid sick leave (depending on work location).

Key Responsibilities:

• Conduct outbound calls to members to support care plan next steps and address questions or concerns.
• Provide outreach to connect members with community or health plan resources.
• Serve as the front-line support for various member and provider inquiries, explaining care plan procedures and protocols.
• Support member onboarding and day-to-day administrative duties, including sending welcome letters and educational materials.
• Document and maintain non-clinical member records to ensure compliance with state and regulatory requirements.
• Address Social Determinants of Health (SDOH) needs by making appropriate referrals.
• Comply with all policies and standards while performing other duties as assigned

Key Requirements and Technology Experience: 

• 2+ years of experience in managed care, healthcare administration, or working with vulnerable populations, individuals with disabilities, or members with chronic/complex conditions.
• High School diploma or GED.
• Strong communication skills and attention to detail.
