Are you an experienced care manager or supervisor who wants to make a meaningful impact while helping other professionals grow?
New Horizon Counseling Center is looking for a Unit Supervisor to lead a team of Health Home Care Managers serving clients throughout Nassau and Suffolk Counties.
This is an opportunity for someone who enjoys both leadership and direct client engagement. You'll supervise and mentor care managers, provide guidance on complex cases, and remain connected to the clients and communities we serve.
Our clients include individuals living with Serious Mental Illness (SMI), Substance Use Disorders, frequent hospitalizations, and housing instability, including individuals residing in shelters.

What You'll Do
Lead & Mentor a Team
- Supervise and support a team of Care Managers.
- Provide regular individual and team supervision.
- Coach staff through challenging cases and professional situations.
- Conduct case reviews and help staff develop effective interventions and care plans.
- Support staff development, training, and performance.
- Help staff navigate high-risk situations, safety planning, and incident reporting.
Stay Connected to Clients
- Participate in community-based client visits throughout Nassau and Suffolk.
- Visit clients in homes, shelters, hospitals, and other community settings.
- Help ensure clients receive appropriate follow-up after ER visits and hospitalizations.
- Advocate for clients and help address barriers to care.
- Monitor client satisfaction and ensure services are meeting client needs.
Keep the Team Organized & Accountable
- Review progress notes, assessments, and care plans.
- Maintain quality documentation in the EHR.
- Manage staff schedules, caseload assignments, and documentation.
- Ensure compliance with Health Home policies and clinical standards.
- Use Outlook, Word, Excel, and other systems for communication and reporting.
Build Community Partnerships
- Collaborate with shelters, hospitals, healthcare providers, and social service organizations.
- Strengthen relationships with community partners.
- Assist with crisis intervention and client advocacy when needed.
What We're Looking For
Required:
- Bachelor's degree in Social Work, Psychology, Sociology, Education, Nursing, Human Services, Healthcare Management, or a related field. Master's degree strongly preferred.
- At least 3 years of experience in case management, care coordination, or a related field.
- Experience working with individuals with SMI and/or individuals experiencing homelessness or housing instability.
- Strong communication, organization, leadership, and problem-solving skills.
- Ability to work independently while collaborating effectively with a team.
- Comfortable traveling throughout Nassau and Suffolk for client and staff support.
Preferred:
- Experience with Health Home Care Management.
- Experience working with shelters, hospitals, behavioral health providers, or community-based organizations.
