    The Care Integration Senior Aide (CISA) conducts in-home visits to work directly with clients and Direct Support Professionals (DSPs). The CISA observes, documents, and reports changes in clients’ health and social conditions, collaborates with the care team to help prevent avoidable negative outcomes, and supports quality service delivery throughout the Community Supported Living (CSL) program.

    • Conductin-homevisitstoobserveanddocumentclients’healthandsocialneeds.
    • SupportandcoachDirectSupportProfessionals(DSPs)inservicedeliveryandclientengagement.
    • Communicateobservations,concerns,andupdatestotheinterdisciplinarycareteam.
    • Completefollow-upactivities,maintainmemberrecords,andsupportcareplanimplementation.
    • TrainandguideDSPsonapprovedsupportsandevolvingclientneeds.
    • Buildpositiverelationshipswithclients,families,caregivers,andcommunitypartners.
    • ServeasaresourcetoDSPsandpromoteperson-centered,high-qualitycare.
    • Adheretoagencypolicies,professionalstandards,anddocumentationrequirements.
    • ProvidedirectsupporttoCSLmembersasneededwithinthescopeoftherole.

    This position provides guidance and support to DSPs but doesnothavesupervisoryresponsibilities,includingscheduling,performancemanagement,ordisciplinaryfunctions.

    Qualifications

    • High school diploma or equivalent.
    • One year of caregiver experience with the agency preferred, or two years of caregiver experience in a similar setting.
    • Reliable transportation, a valid driver’s license, and proof of current automobile insurance.
    • Ability to speak, read, and write functional English sufficient to perform essential job duties and complete required documentation.
