Pay Range: $22.5 - $23/hour. Employee benefits include, but are not limited to, health insurance (medical, dental, vision), 401(k) plan, and paid sick leave (depending on work location).
Key Responsibilities:- Perform member outreach telephonically or through home visits to engage members and discuss care/service plans.
- Coordinate care activities based on the care plan/service plan and work with healthcare providers and community partners.
- Serve as a liaison alongside care managers and providers to ensure proper coordination of care.
- Document and maintain member records in accordance with state and regulatory requirements.
- Provide education on benefits and resources available to members.
- Assist with training and development needs as necessary.
- Comply with all policies and standards, ensuring adherence to contractual requirements and regulatory guidelines.
Key Requirements and Technology Experience:- 2 - 4 years of related experience in healthcare record review, auditing, medical record abstraction, care management documentation, HEDIS, LTSS, Medicaid, or HCBS.
- High School diploma or GED required.
- Bachelor’s degree in nursing, social sciences, social work, or related field preferred.
- One (1) year of supervised clinical experience in a human-services field preferred.
- Must reside in Illinois, Florida, or Arkansas depending on the specific care plan.
