Description:
The Care Coordinator provides both clinical and clerical support within a colleges clinical areas. Ensures the provision of quality and compassionate evidence-based care in a highly integrated environment. Accountable for collaborating with the care team to consistently communicate and document high-risk patient profiles while ensuring continuity of care for all patients. Works closely with the care team to connect the dots of collaborative patient care. Assists patients in navigating insurance and financial assistance programs, connecting them to resources, arranging transportation, and ensuring follow-up to improve access and outcomes for their medical conditions.

MQ: Education:
Requires specialized training in basic trades, principles, procedures practice, routines or techniques in a specific area or trade which might normally be acquired through up to 18 months of education or training beyond the high school level.
Vocational competence in the operation of mechanical or electronic equipment may be required.
Experience:
Requires a minimum of three (3) years of directly job-related experience.
