Position Overview

The Care Coordinator II at Harvest Family Health Center supports the Population Health program through the provision of care coordination services (e.g., patient reminder calls and mailings, conducting annual health risk and comprehensive risk assessments, assisting with care gap and referral closure, etc.).

THIS POSITION OFFERS A 40-HOUR WORKWEEK, NO WEEKENDS, FULL BENEFITS PACKAGE, 12 PAID HOLIDAYS, BI-WEEKLY PAY.

Essential Tasks

  • Contact patients lost to care through phone, letter, or the patient portal and encourage them to return to care.

  • Assess provider schedules for appointment availability, assist with phone call reminders, and provide outreach to patients to schedule needed examinations.

  • Close the referral loop by obtaining radiology and specialty reports/office visit notes, marking the referral closed in the referral workqueue, and entering results in the electronic medical record.

  • Track and provide outreach to prenatal care patients until the time of delivery.

  • Assist with comprehensive risk assessments for identified patient populations. The results are recorded and communicated to the case managers and medical provider as directed.

  • Assist medical providers in the completion of forms for insurance prior authorizations and medical supplies.

Experience and Education

  • Associate''s degree in Health Information Technology, Healthcare Administration or equivalent education, or Pharmacy Technician II certification with one year experience; or five years of experience as a Care Coordinator I and certification as a nursing assistant.
  • Graduate from an accredited program for nursing assistants and certification.
  • Effective communication and interpersonal skills.
  • Must be organized and detail-oriented.
  • Ability to maintain confidentiality.
  • Must be team-oriented.
  • Basic computer skills.

Monday, Tuesday, Wednesday, Thursday, Friday

8 am - 5 pm

Physical Requirements

  • Individuals may need to sit or stand for long periods of time, occasional bending, stooping, lifting including of patients, reaching forwards and overhead.
  • Possibly requires walking primarily on a level service for periods throughout the day.
  • Both proper lifting techniques and frequent computer work are required.
  • Must be able to travel freely in the local area; between clinics, hospitals, health departments, etc.

Base Pay Overview
