Description

Job Title: Care Coordinator II

Contract Type: Contract | 6 Months (Potential Extension)

Location: Remote Kentucky Residents Only

Work Hours:

  • Training: Monday Friday, 9:00 AM 5:30 PM (EST/CST)
    • 2 weeks classroom training + 1 week one-on-one training
    • Cameras must remain on during training
    • No time off permitted during training
  • Regular Schedule:
    • Monday Friday, primarily 9:00 AM 5:30 PM EST/CST
    • Schedule may flex based on business needs
    • Temp workers will be asked to go in person to staff meetings once a quarter at Centene hub - Louisville or Lexington

      Not a requirement of the job right now but if hired full-time they will be expected to go to community events as needed. Will be doing digital eye exams

Pay Rate: $20 23/hour (Based on Certifications)

Key Responsibilities
  • Conduct outbound member outreach to support preventive health initiatives
  • Educate members regarding healthcare services and available resources
  • Schedule appointments with providers
  • Document all member interactions accurately while speaking with members
  • Coordinate follow-up care and social determinant of health (SDOH) resources
  • Support outreach campaigns for:
    • Well-child visits
    • Postpartum care
    • A1C testing
    • Breast cancer screenings
  • Meet productivity and quality goals while delivering excellent member service
Education
  • High School Diploma or GED required
Preferred Certification
  • Community Health Worker (CHW)
Employment Requirements
  • Must reside in Kentucky
  • Candidate must be eligible for W-2 employment
  • No Corp-to-Corp (C2C) arrangements
  • Must be legally authorized to work in the U.S. without current or future sponsorship
