This role serves as the central point of contact ensuring medical needs, regulatory compliance, and care continuity for all persons served.Scope of ResponsibilityState Medicaid POC and authorization managementABI Waiver Medicaid POC and authorization managementMedical appointment scheduling and coordinationMedication management coordination with RN Executive DirectorADT Team supervisionCase management and guardian communicationsProvider communication and follow-upsInterdisciplinary team coordinationWhat This Role OwnsMedicaid & Authorization Management: All State Medicaid and ABI Waiver authorizations, submissions, and trackingMedical Coordination: Scheduling and completion of all medical appointmentsCase Management: Primary point of contact for case managers and guardians on all casesTeam Supervision: Oversight of ADT Team operationsClinical Support: Coordination with RN Executive Directors on medication management and program supportTransportation Logistics: Arrangement and scheduling of medical transportProvider Follow-Up: Tracking of provider recommendations and pending documentationCommunication Hub: Medical updates across all departments and stakeholdersRecord Accuracy: Medical coordination and authorization documentationCore AccountabilitiesMedicaid authorization compliance and timelinessMedical appointment coordination and completionADT Team performance and workflowProvider and guardian communicationInterdisciplinary team collaborationTracking and follow-up systemsDocumentation accuracy and audit-readinessKey ResponsibilitiesMedicaid & Authorization ManagementManage all State Medicaid Plan of Care (POC) and authorization processesOversee ABI Waiver Medicaid POC and authorization managementEnsure timely submission, tracking, and renewal of all authorizationsMaintain compliance with Medicaid requirements and deadlinesMedical Appointment CoordinationSchedule and track all medical, dental, and specialty appointmentsCoordinate transportation and logistics for all medical appointmentsCommunicate appointment details to staff and ensure attendanceTrack missed appointments and reschedule promptlyEnsure provider recommendations are communicated to nursing and leadershipMaintain comprehensive logs of appointments, follow-ups, and outcomes Clinical CollaborationWork directly with RN Executive Director on medication management for residential homesProvide coordination and support to RN Executive Director #2 for Frankfort program operationsEnsure clinical protocols are followed across all service locationsCase Management & CommunicationServe as primary Point of Contact for all case managers and guardiansManage all case-related emails and interdisciplinary team communicationsFacilitate coordination between clinical, residential, ADT, and community teamsFollow up on pending medical documentation or provider responsesTeam SupervisionSupervise ADT TeamCoordinate workflow between ADT Team, transport personnel, and ADT PRN staffEnsure seamless patient transitions and transfersDocumentation & ComplianceEnsure all records are organized, accurate, and accessible for auditsMaintain tracking systems for authorizations, appointments, and follow-upsSupport documentation needs across departmentsOther DutiesPerform other duties as assigned by RN Executive DirectorPerformance MetricsAuthorization submission and approval timelinessAppointment completion rateMissed appointment rate and resolution timeTimeliness of provider follow-upsAccuracy of tracking logs and documentationCommunication effectiveness with stakeholdersADT Team performance indicatorsCase manager/guardian satisfactionMinimum QualificationsHigh school diploma required; Associate's or Bachelor's degree preferredExperience in healthcare coordination, case management, or Medicaid authorization preferredKnowledge of Medicaid waiver programs (ABI Waiver experience a plus)Strong organizational and multitasking abilitiesExcellent written and verbal communication skillsProficiency with healthcare documentation systemsAbility to supervise and coordinate team activitiesDetail-oriented with strong follow-through capabilitiesPreferred QualificationsExperience with ABI (Acquired Brain Injury) populationsPrevious supervisory experienceFamiliarity with interdisciplinary team coordinationKnowledge of transportation coordination for medical services
