Care Coordinator Highlights

Pay: $20.00 per hour
Schedule: Must be able to work any 8-hour shift between 7:00 AM – 7:00 PM (CST) as assigned (40 hrs/week)
Work setting: 100% Remote / Work From Home
Employment type: Temporary (approx. 16 weeks), potential for full time employment
Training: 8:00 AM – 5:00 PM (CST) for the first 4–6 weeks (virtual attendance mandatory)
Responsibilities

Create and complete accurate referrals and applications in a centralized CRM platform; stay current on policy/procedure updates
Resolve patient questions and concerns regarding patient journey status
Enter detailed, accurate, and clear information into proprietary systems while speaking with patients by phone
Place outbound calls for patient follow-ups and confirmations
Provide an empathetic, supportive patient experience while guiding and controlling the conversation appropriately
Interact with referral sources (including healthcare providers) to process new applicants
Steward patient accounts from initial contact through final approval/denial
Apply job skills and standard procedures to complete routine assignments and basic problem resolution; escalate complex items as needed
Requirements

Ability to work a flexible assigned 8-hour shift between 7 AM–7 PM CST
Ability to attend required training 8 AM–5 PM CST for 4–6 weeks
Comfortable handling high-volume phone work while accurately documenting in multiple systems
Strong attention to detail, professionalism, and patient-focused communication
