Care Coordination Case Manager

Essential Responsibilities:

Identify and engage individuals eligible for Care Coordination services through targeted outreach. Serve as the primary point of contact, coordinating care among providers, families, and other involved parties. Maintain ongoing contact with individuals and connect them to appropriate treatment, support, and recovery services. Complete biopsychosocial and needs assessments in accordance with the Wraparound model. Document assessment findings, referrals, services provided, and progress in a timely and accurate manner. Prepare and submit required reports and documentation to the Supervisor and Qualified Professional within established deadlines. Collaborate with internal and external partners to support continuity of care and positive outcomes. Travel within the service area as needed to provide outreach and care coordination services.

Requirements:

Bachelor's degree in Social Work, Psychology, Human Services, Counseling, or a related field preferred. Experience working with individuals experiencing behavioral health, substance use, or co-occurring disorders preferred. Knowledge of care coordination, case management, recovery support services, and community resources. Ability to conduct biopsychosocial assessments and develop individualized service plans. Strong interpersonal, communication, and organizational skills. Ability to work effectively with individuals, families, service providers, and community partners. Proficient in documentation, data entry, and maintaining accurate client records. Knowledge of the Wraparound model and trauma-informed practices preferred. Ability to manage multiple priorities and meet established deadlines. Valid driver's license, reliable transportation, and ability to travel within the service area.

  • Strong Communication Skills: Ability to build rapport, actively listen, and communicate effectively with individuals, families, providers, and community partners.
  • Compassion and Empathy: Demonstrates understanding and sensitivity toward individuals facing behavioral health, substance use, or other personal challenges.
  • Organization and Time Management: Effectively manages multiple cases, documentation requirements, deadlines, and follow-up activities.
  • Problem-Solving Skills: Identifies barriers to care, develops solutions, and connects individuals with appropriate resources and services.
  • Collaboration and Teamwork: Works well with multidisciplinary teams and community agencies to ensure coordinated, person-centered care.

Salary Rate

$20.00 per hour
