    Overview

    We are seeking a compassionate and community-focused Care Coach I to support members in achieving and maintaining optimal health and wellness. This role involves assessing member needs, coordinating care, removing barriers to treatment, and connecting individuals with appropriate healthcare and community resources.

    This is a highly field-based position requiring 75 90% local travel to members homes, healthcare facilities, and community locations throughout the Highlands County area, including Sebring and Avon Park.

    Responsibilities

    • Conduct assessments to identify member needs, barriers to care, and opportunities for improved health outcomes
    • Develop and implement individualized care plans in collaboration with members, families, providers, and community resources
    • Educate and coach members on health management, wellness, and available support services
    • Coordinate care and facilitate access to healthcare, social services, and community-based programs
    • Monitor member progress through ongoing assessment, documentation, and follow-up
    • Identify and resolve barriers that may impact treatment adherence or overall wellness
    • Maintain accurate case documentation and member records using electronic systems
    • Collaborate with healthcare providers, caregivers, and interdisciplinary teams to support continuity of care
    • Travel regularly to member homes, facilities, and community locations throughout the assigned territory

    Required Qualifications

    • Bachelor s degree
    • Minimum of 2 years of related healthcare, social services, care management, or community support experience
    • Knowledge of Microsoft Word and Excel
    • Strong verbal, written, and interpersonal communication skills
    • Experience using electronic documentation systems and software applications
    • Ability to work independently and manage a field-based caseload
    • Valid driver's license
    • Proof of personal vehicle liability insurance meeting required coverage limits
    • Ability to travel locally up to 75 90% of the time
    • Willingness to participate in required health screenings

    Preferred Qualifications

    • Master s degree
    • Experience working with Medicare and Medicaid populations
    • Experience with care management, health coaching, wellness programs, or population health initiatives
    • Experience documenting in multiple systems and maintaining electronic case notes
    • Knowledge of community health resources, social service agencies, and local support programs
    • Background in healthcare, case management, behavioral health, social work, or community outreach
