Job Summary and Qualifications Responsible for the direct and indirect care of patient assignment and for working collaboratively with assigned staff. Provide patient/disease specific education to the cardiopulmonary patient for promoting risk factor modification and behavior change. Follow hospital policies and procedures. · Demonstrate critical thinking skills, scientific judgment and leadership in the anticipation and planning for the departmental scope of service. · Assume responsibility and accountability for the management of the cardiopulmonary patients during their rehabilitation. · Evaluate new and current patients in regard to their current health status. · Complete and document on ITP for new and current patients, initial evaluation, reassessments and discharge (including nursing assessment, establishment of goals, initiating standards of care and outcomes management). · Monitor patients on EKG monitor during their exercise sessions, utilize telemetry monitoring system per policy and procedure and recognize and interpret arrhythmias. · Provide patient education specific to cardiopulmonary disease process, risk factor modification and promoting behavior change. · Participate in interviewing, monitoring, coaching, evaluating and counseling. · Prescribe exercise programs according to their knowledge of fitness, the participants' goals, physiologic data and physicians' recommendation. · Proficient in use of equipment, including exercise, oxygen and oximetry. Orient new participants and evaluate exercise programs. Monitor and supervise exercise sessions. · Assist with maintenance and cleaning of exercise equipment, scheduling, billing, mailing and other office tasks as needed.
