US Heart and Vascular is needing a Cardiac Rehabilitation Nurse Coordinator to join our team at Orion Medical in Houston, TX
Position Summary
The Cardiac Rehabilitation Nurse Coordinator is responsible for the clinical coordination, daily operation, and patient care activities of the Cardiac Rehabilitation and Intensive Cardiac Rehabilitation (ICR) Program. The Coordinator provides patient assessment, telemetry monitoring, education, care coordination, and lifestyle modification coaching while serving as a key liaison among patients, physicians, and the multidisciplinary rehabilitation team.
This position promotes evidence-based cardiovascular risk reduction and secondary prevention strategies designed to improve functional capacity, reduce hospital readmissions, and enhance overall cardiovascular health.
Responsibilities & Duties
Program Coordination
Coordinates daily operations of the Cardiac Rehabilitation and Intensive Cardiac Rehabilitation programs.
Manages patient enrollment, scheduling, orientation, and progression through the program.
Ensures all patients meet established eligibility and admission criteria.
Facilitates communication regarding patient progress and outcomes among referring physicians, cardiologists, primary care providers, and rehabilitation staff.
Coordinates interdisciplinary team meetings and patient case reviews.
Assists leadership with program development and strategic growth initiatives.
Clinical Assessment and Patient Care
Performs comprehensive initial and ongoing nursing assessments.
Obtains and documents:
Medical history
Cardiovascular diagnoses
Surgical and procedural history
Medication profiles
Risk factors for cardiovascular disease
Psychosocial assessments
Functional capacity measurements
Develops individualized patient goals and treatment plans in collaboration with the ASC Director and rehabilitation team.
Monitors patient progress and modifies care plans as needed.
Exercise Monitoring and Safety
Monitors patients during exercise sessions using telemetry and clinical assessment, including blood pressure, heart rate, oxygen saturation, and symptom response.
Evaluates ECG rhythms and recognizes arrhythmias and other clinically significant events.
Responds appropriately to emergencies and activates emergency response procedures when necessary.
Maintains ACLS competency and participates in emergency preparedness activities.
Ensures emergency equipment is operational and available.
Patient Education and Lifestyle Coaching
Provides individualized and group education regarding:
Coronary artery disease
Heart failure
Hypertension
Hyperlipidemia
Diabetes management
Weight management
Smoking cessation
Stress reduction
Medication adherence
Exercise and physical activity
Heart-healthy nutrition
Uses motivational interviewing techniques to:
Promote lifestyle modification
Improve patient engagement
Enhance program completion rates
Support long-term behavioral change
Physician and Community Relations
Maintains professional relationships with referring physicians and cardiology practices.
Assists with community outreach and education efforts.
Supports physician recruitment and referral development initiatives.
Quality Improvement and Outcomes Management
Participates in quality improvement activities.
Tracks, analyzes, and reports key performance indicators, including:
Program enrollment
Attendance rates
Completion rates
Functional capacity improvement
Blood pressure control
Weight management outcomes
Patient satisfaction metrics
Assists with preparation of quarterly and annual program reports.
Compliance and Accreditation
Ensures compliance with:
Medicare and commercial payer requirements
State nursing regulations
Program policies and procedures
Applicable accreditation standards
Maintains documentation required for audits and surveys.
Assists with accreditation readiness activities.
Supports development and review of program policies and clinical protocols.
Requiremnents:
Bachelor of Science in Nursing (BSN) from an accredited school of nursing.
Current Registered Nurse license in the state of practice.
Current BLS certification.
Current ACLS certification.
Certified Cardiac Rehabilitation Professional (CCRP) is preferred but not required
Experience with CMS Intensive Cardiac Rehabilitation programs is preferred but not required
