Cardiac Rehab Nurses are specialized registered nurses work with patients recovering from heart-related conditions or surgeries, such as heart attacks, bypass surgery, or angioplasty. They play a key role in helping patients regain strength, adopt heart-healthy habits, and reduce the risk of future cardiac events. Their responsibilities include monitoring patients' progress, providing education on lifestyle changes, and collaborating with doctors and other healthcare professionals to create individualized rehabilitation plans.
· Assists with physician rounding to ascertain plan of care for patients and provide update on patients.
· Assists with development of Individual Treatment plans and documentation of pre and post outcomes in LSI system.
· Monitors Core Measure metrics. Ensuring staff are aware of requirements.
· Maintains reporting responsibility to Department Clinical Coordinator/Supervisor and Director
· Oversees completion of unit specific administrative tasks (e.g. census, EOC audits, narcotic counts, etc.)
· Participates in decision making regarding patient flow (ad/d/c/transfers). Adapts work plan to change in unit workload.
· Actively participates in performance improvement activities, completion of assignments, and shared governance.
· Addresses patient/family concerns/satisfaction issues. Informing department leadership of concerns.
· Informs Nursing Supervisor of any unit issues and in real time regarding bed placement.
· Engaged in patient satisfaction. Discusses and monitors responsiveness, pain and nurse communication.
· Validates intentional rounding performed by staff and provides recognition/coaching as needed. Reports to Director staff who continuously are in need of coaching.
· Conduct chart audits for documentation compliance.
· Assists with the interview process of new hires.
· Ensure compliance with AACVPR Accreditation requirement.
· Accepts departmental accountability for current and new policies, clinical programs and systems necessary to support changing patient care needs.
· Coordinate nursing care with other disciplines to maximize outcomes and resource allocation.
· Speaks positively about the organization and demonstrates positive attitude and behavior in the work environment.
