Marin Health Medical Center is seeking a temp Director of Care Coordination. Qualified Candidate must meet the following qualifications:

• BSN required, master's degree in nursing, Health Administration or Business Administration is preferred.
• Case Management certification (e.g., CCM or ACM) preferred.
• Minimum five (5) years of experience in an acute healthcare setting required.
• Minimum five (5) years of progressively responsible leadership/administrative experience in Case Management, Care Coordination, Utilization Management, or a closely related function required.
• Demonstrated experience leading interdisciplinary clinical/operational programs, staff, performance improvement, and regulatory or accreditation readiness.
• Advanced knowledge of case management, care coordination, utilization management, discharge planning, patient progression, and transitions of care in an acute-care environment.
