Job Description:

Marin Health Medical Center is seeking a temp Director of Care Coordination. Qualified Candidate must meet the following qualifications:BSN required, master's degree in nursing, Health Administration or Business Administration is preferred.Case Management certification (e.g., CCM or ACM) preferred.Minimum five (5) years of experience in an acute healthcare setting required.Minimum five (5) years of progressively responsible leadership/administrative experience in Case Management, Care Coordination, Utilization Management, or a closely related function required.Demonstrated experience leading interdisciplinary clinical/operational programs, staff, performance improvement, and regulatory or accreditation readiness.Advanced knowledge of case management, care coordination, utilization management, discharge planning, patient progression, and transitions of care in an acute-care environment.
