About Us

We are fixing US healthcare by building an AI-native physical care platform, starting with home health.

Roughly 40% of what home health spends goes to administrative work, not patient care. That cost structure is why agencies have to turn away patients they know they could help. We automate that work, which lets us say yes to the roughly 30% of patients who go untreated today.

It also changes the job. We have built branch clinical leadership to be free from high-volume, process-intensive back-office functions, with centralized support behind them. That lets our clinical leaders put their full focus where it matters most: active clinical oversight, skills training and development, and driving the systems and behaviors that keep patients safe and out of the hospital. The same technology removes administrative burden from field clinicians so they can do what they were trained to do.

Position type: Onsite, branch-based leadership role in the Detroit metro. This is an active, high-intensity clinical oversight position, not a desk role.

Position Overview

We are looking for a Clinical Manager who can lead clinical execution with high standards and high intensity.

This is not a passive clinical support role. The branch Clinical Manager owns day-to-day clinical quality, staff development, and rehospitalization prevention for their branch. You are accountable for outcomes, not just activities. You will partner closely with operations, scheduling, and leadership to keep care delivery on track, and you will be a key leader in how we scale safely.

What You'll Do

Own clinical execution for your branch

  • Provide direct clinical guidance to clinicians as needed

  • Respond to clinical calls from patients, caregivers, and providers, and identify actions needed with appropriate clinical judgment

  • Monitor daily on-call events to identify urgent patient status changes

  • Conduct real-time evaluation reports from assessing clinicians to identify immediate patient needs, ensure the care plan is complete, provide guidance on visit utilization, and follow up on verbal plan of care approval so physician orders are timely and complete

  • Complete transfer reviews daily to assess hospitalized patients, including root cause analysis and action planning where improvement is needed

  • Review high-risk patients and coordinate proactive intervention to prevent avoidable events

Lead and develop field clinicians

  • Coach clinicians on clinical reasoning, visit planning, patient education, and documentation quality

  • Address performance issues quickly and fairly, including structured improvement plans when needed

  • Lead new clinician orientation and skills validation to ensure competency before independent deployment

  • Build your own clinical team as the branch grows

Lead quality and compliance

  • Own branch survey readiness: documentation quality, clinical compliance, and field staff preparedness

  • Track HHCAHPS results and quality KPIs, identify trends, and implement coaching or process changes

  • Partner with our centralized Quality Assurance team on documentation accuracy and OASIS review

  • Perform field supervisory visits covering infection control, safety, compliance, and clinical skills

Be the escalation leader

  • Make calm, decisive escalation decisions for high-risk patients, safety concerns, and incidents

  • Coordinate with leadership on sensitive patient situations, hospitalizations, and quality investigations

Who You Are

A high-accountability clinical leader who loves developing clinicians. You bring urgency, clarity, and standards to complex clinical operations. You are energized by coaching clinicians, solving care delivery problems in real time, and holding the line on quality every day.

Must-haves

  • Active, unrestricted RN license valid in Michigan. Michigan does not participate in the nurse licensure compact, so an out-of-state license does not satisfy this.

  • Minimum one year of leadership experience at a Medicare-certified skilled home health agency, with direct reports

  • Ability to coach, mentor, and train clinical staff across skill levels

  • Strong clinical judgment and a structured approach to prioritization and escalation

  • A strong sense of personal accountability. You own clinical outcomes through your clinical staff.

  • Ability to communicate clearly across clinical and operational teams

  • Willingness and ability to be present onsite at the branch as an active clinical leader

Nice-to-haves

  • Experience opening or scaling a new branch or market

  • Experience at an agency with centralized or shared-services back office functions

  • Experience partnering closely with operations teams to prevent service failures

  • Experience leading new hire clinical orientation and skills validation programs

Compensation and Benefits

  • Pay: Base + bonus. On Target Earnings (OTE) $115K-$135k

  • Benefits: Health, dental, and vision insurance. 401(k) with employer match. Paid time off.

Working Conditions

  • Primarily branch-based with regular field responsibilities including supervisory visits. Local travel and patient home visits as needed. Must be able to manage multiple priorities in a fast-paced environment. Occasional exposure to varied home environments and weather conditions. Must be able to lift up to 50 lbs and support patients as needed.

Work location: Onsite, Detroit metro.
