    We’re seeking a Licensed Practical Nurse (LPN) to support patient care through chronic disease management, triage, and care coordination. This role focuses on high-risk patient populations and works closely with providers and care teams to deliver patient-centered, team-based care across primary care, women’s health, and pediatrics.

    What You’ll Do
    • Provide triage (phone and in-person)
    • Manage chronic care patients and support population health initiatives
    • Educate patients and families on chronic conditions and treatment plans
    • Coordinate follow-ups, referrals, and transitions of care
    • Support women’s health services, including prenatal/postpartum care and patient education
    • Administer vaccines, perform point-of-care testing, and complete nurse visits
    • Collaborate with providers and interdisciplinary care teams
    • Participate in case management, quality initiatives, and community outreach
    • Maintain accurate documentation and ensure HIPAA compliance
    Schedule Expectations
    • Full-time role with travel between LCH locations as needed
    What You Bring
    • Bilingual English/Spanish required
    • Minimum of Associate's in nursing
    • Current or eligible LPN license in Pennsylvania
    • CPR certification required
    • Community or public health experience (preferred)
    • Experience working with diverse populations (preferred)
    • Experience with EHR systems and Microsoft Office

    • Organized, detail-oriented, and able to multitask
    • Experience handing multiple priorities in a team-based environment
