Immediate need for a talented Bilingual Long-Term Care Service Care Manager. This is a 06+months contract opportunity with long-term potential and is located in U.S(Remote). Please review the job description below and contact me ASAP if you are interested.

Pay Range: $25 - $29/hour.  Employee benefits include, but are not limited to, health insurance (medical, dental, vision), 401(k) plan, and paid sick leave (depending on work location).

Key Responsibilities:

• Manage a caseload of 50-70 members, focusing on geriatric long-term care.
• Conduct assessments with members, caregivers, or providers to develop care plans.
• Monitor service delivery and follow up with members through in-person visits and telephonic contact.
• Authorize and coordinate referrals for services, ensuring no gaps in care.
• Collaborate with discharge planners, physicians, and other stakeholders for effective care coordination.
• Assist members with filing and resolving complaints and appeals.
• Maintain compliance with new contract requirements effective 2/1/25.

Key Requirements and Technology Experience:

• Key Skills; Experience with discharge planning.
• Familiarity with TruCare software.
• 2+ years of care management experience, with a strong emphasis on field experience.
• Bachelor's degree in a relevant field (e.g., Psychology, Sociology, etc.).
• Valid Driver’s License.
• Experience managing high caseloads in a fast-paced environment.
• Proficiency with electronic medical health records.
• Bilingual in Spanish is required.
