Bilingual Family Navigator

Location: Tulsa, OK
Pay: $17.26/hour
Schedule: Monday–Friday, 8:00 AM–5:00 PM
Employment Type: Full-Time, Direct Hire

Position Summary

We are seeking a compassionate and bilingual Family Navigator to help expectant mothers, parents, caregivers, and families connect with healthcare, early childhood, and community resources. This position manages an assigned caseload, coordinates referrals, provides education, and builds trusting relationships with families. The ideal candidate is organized, dependable, culturally sensitive, and passionate about helping families access the support they need.

Essential Duties & Responsibilities

• Build trusting relationships with expectant mothers, parents, caregivers, and families from diverse backgrounds.
• Manage an assigned caseload and maintain regular communication by phone, text, email, and in person.
• Connect families with prenatal, postpartum, home visitation, early childhood, healthcare, education, and social service programs.
• Assess family needs and coordinate appropriate referrals to healthcare providers, social workers, case managers, and community agencies.
• Educate families about prenatal and postpartum resources, early childhood development, parenting support, and available services.
• Follow up on referrals and help families successfully navigate available programs and resources.
• Maintain accurate and timely documentation of client contacts, referrals, services, and outcomes.
• Serve as a liaison between families, healthcare providers, community organizations, and social service agencies.
• Develop relationships with community partners and identify additional resources for families.
• Participate in team meetings, community events, networking opportunities, and required training.
• Maintain current knowledge of community resources, family support programs, and health-related social needs.
• Maintain strict confidentiality and comply with HIPAA and applicable privacy requirements.
• Collaborate with team members on outreach, referral coordination, and special initiatives.
• Perform other duties as assigned.

Qualifications / Requirements

• High school diploma or equivalent required.
• Bilingual English/Spanish required.
• Previous experience working with families, children, healthcare, social services, community programs, case management, or patient navigation preferred.
• Valid Oklahoma driver's license required.
• Reliable transportation and willingness to travel locally for home visits, community events, meetings, and training.
• Basic computer skills, including email, electronic documentation, Microsoft Office, and smartphone applications.
• Ability to maintain accurate and confidential records.

Preferred Qualifications

• Associate degree or higher in social services, human services, early childhood education, healthcare, public health, or related field.
• Experience working with underserved or culturally diverse populations.
• Experience with home visitation, family support, community health, care coordination, or case management.
• Knowledge of Tulsa-area healthcare, social service, childcare, and community resources.

Work Environment & Physical Requirements

Primarily office-based with regular local travel for home visits, community events, meetings, and training. Must be comfortable using a computer and telephone for extended periods and communicating with families in person, by phone, text, and email.

Additional Requirements

Background screening may be required. Must complete required training and maintain confidentiality of client information. No regular weekend or on-call schedule is expected.
