Key Responsibilities

  • Communicate with patients and families to assess care needs and provide ongoing support
  • Develop, coordinate, and manage individualized patient care plans
  • Schedule and coordinate appointments and services with healthcare providers
  • Educate patients and families on medications, conditions, and care instructions
  • Monitor patient progress and maintain accurate documentation
  • Collaborate with nurses, aides, and healthcare professionals to ensure quality care delivery
  • Maintain professionalism, empathy, and excellent customer service at all times

Qualifications

  • Minimum 3 years of experience in care coordination, healthcare administration, or clinical practice
  • Bilingual in English and Bengali required
  • Strong communication, organizational, and multitasking skills
  • Proficiency in Microsoft Office and general computer systems
  • Ability to work in a fast-paced healthcare environment
