BHCM II Promise Care Manager

Location: Newark, DE 19702
Work Setting: On-site
Schedule: Monday Friday, 1st Shift
Hours: 37.5 hours/week
Contract: 1 year

Job Summary

The Promise Care Manager will work as part of an interdisciplinary behavioral health team serving individuals with severe and persistent mental illness (SPMI), including individuals with co-occurring disorders.

The ideal candidate will have experience in behavioral health case management, care coordination, mental health services, treatment/recovery planning, assessments, and community-based support.

Key Responsibilities

  • Perform behavioral health assessments and identify client needs.
  • Help develop and implement individualized recovery plans.
  • Determine appropriate levels of care and assist with placement.
  • Coordinate and monitor services to ensure care is delivered according to established standards.
  • Conduct community visits with clients/beneficiaries.
  • Maintain accurate and timely clinical documentation and charts.
  • Coordinate services with community-based providers and agencies.
  • Serve as a liaison between clients and community providers.
  • Collaborate with an interdisciplinary team of behavioral health professionals.
  • Support clients with accessing appropriate community resources and services.
  • Work with individuals experiencing serious mental illness and/or co-occurring disorders.

Minimum Qualifications

  • Bachelor's Degree required.
  • Valid Driver's License required.
  • Must be able to operate a vehicle for required community travel.
  • Behavioral health, mental health, case management, or care coordination experience is highly preferred.

Work Environment

This is an onsite, Monday Friday, first-shift position based at the New Castle Promise Assessment Center in Newark, Delaware. The position also requires community travel, so candidates must have a valid driver's license and be able to operate a vehicle.
