Description

This position is on the Youth and Family Team and located at the Sharon Bulova Center in Fairfax, VA. Provides behavioral health services to youth and families while collaborating with internal and external service professionals and community partners. Also provides case management, outreach, engagement, wellness and health promotion, crisis stabilization, crisis management, risk assessments, and entry, navigation, and referral services. Maintains accurate and comprehensive electronic health records in accordance with federal, state, and department regulations. Advocates for clients regarding their rights and needed services. Develops and maintains effective relationships with community organizations and agencies. Assesses and monitors the impact of medications and administering medications as ordered by a physician in accordance with department regulations. Also supports clients and families in accessing appropriate behavioral health and community resources. 

The successful candidate will have the ability to manage a caseload, establish rapport and maintain effective relationships with clients and families, collaborate effectively with coworkers and community partners, communicate clearly both orally and in writing, maintain accurate records, and prepare reports.
