Responsibilities:
The social worker will conduct assessment to determine patient needs and provide support by developing and implementing individualized treatment.
The social worker will provide counseling and support to patients and their families, focusing on accessing additional resources and addressing mental health needs. The social worker with monitor and evaluate patient progress and adjust treatment plans as necessary. They will collaborate with multidisciplinary team to provide comprehensive and passionate care.
The provider will serve as a role model for Mercy’s mission and values-is caring, ethical, fair, respectable, compassionate, courteous and hospitable.
Qualifications:
Minimum of LSWA, LGSW or LCSW. LCSW-C preferred
Preferred skill set includes:
Accurate verbal and written expression
Work organization, problem solving Psycho-social counseling
Conflict resolution Identification of crisis situations
Experience with Bio-psychosocial assessments
Experience with Crisis counseling
Experience with Community resource referrals
Experience with Documentation
