Job Description:
Maintains comprehensive community resources and networks to assist patients and families as they transition back to the community. Collaborates with the multi-disciplinary team to establish and document psychosocial determinants of health for safe discharges and to avoid emergency hospital admissions. Serves as a consultant and educator to the multi-disciplinary team regarding psychosocial components of illness and hospitalization. Maintains professional competency by participating in continuing education and attending pertinent conferences and seminars regarding behavioral health. Applies local, state, and federal programs to assist with appropriate referrals and coordination of care. Participates in the development of programs to support patients and families dealing with behavioral health disorders. Attends physician rounds and forums to facilitate communication and behavioral health input to the interdisciplinary team. Follows up with patients in the community and/or post-acute environment to ensure compliance with the hospital discharge plan. Demonstrates working knowledge of the involuntary commitment process. Evaluates for potential safety risks appropriately and responds to potentially dangerous situations to ensure patient safety in the least restrictive manner. Other duties as assigned. Performs behavioral health intakes, psychosocial assessments, care plans, and psychotherapy with patients and families coping with behavioral health disorders.
