The provider will assess and address behavioral health factors that may affect transplant candidacy, readiness, treatment adherence, recovery, and long-term outcomes.Working collaboratively with the multidisciplinary transplant team, the APP will help ensure timely access to behavioral health services while supporting whole-person care for transplant patients and living donors.Essential FunctionsPerform comprehensive psychiatric evaluations and behavioral health assessmentsConduct transplant clearance and readiness evaluationsAssess treatment adherence, substance use, and other behavioral health factors relevant to transplant candidacyDiagnose and manage acute and chronic psychiatric conditionsDevelop individualized behavioral health treatment plansPrescribe and manage psychiatric medications as clinically appropriateProvide behavioral health assessments and support for living donorsMonitor behavioral health needs of patients before and after transplantationUtilize standardized depression, anxiety, and other behavioral health screening tools as clinically indicatedCollaborate with psychiatrists, transplant physicians, surgeons, social workers, transplant coordinators, nursing staff, and other multidisciplinary team membersProvide patient and family education regarding diagnoses, medications, treatment recommendations, and behavioral health resourcesAccurately document assessments, treatment plans, interventions, and patient progress within the medical recordParticipate in transplant-specific quality improvement initiatives and program developmentServe as a patient advocate and support the integration of behavioral health services throughout the transplant continuumPhysical RequirementsFrequent sitting, standing, walking, reaching, and computer useAbility to communicate effectively with patients, families, and healthcare team membersMust possess intact senses of sight, hearing, touch, and finger dexterityMust speak English fluently and clearlyMust be able to respond appropriately and professionally in stressful or crisis situationsWhen you join Atrium Health, you become part of an inclusive culture that values collaboration, innovation, and compassion.
