How you'll make an impact in this role

• Patient Assessment & Care Planning: Conduct initial evaluations to understand patient needs and co-create clear, actionable therapeutic treatment goals.

• Therapeutic Intervention: Deliver direct individual and group counseling sessions, continuously evaluating patient progress and adjusting therapy approaches to ensure maximum effectiveness.

• Collaborative & Integrated Care: Work side-by-side with patients, family members, physicians, and local providers to build a seamless, supportive care team from day one of treatment through discharge.

• Community Connection: Empower patients by connecting them with essential community resources, support networks, and therapeutic activities to extend care beyond the clinical setting.

• Clinical Documentation: Maintain meticulous, timely, and strictly confidential patient records and progress notes in compliance with professional and privacy standards.

What minimum requirements you'll need

Licensure / Certification / Registration:

• One or more of the following required:

• Licensed Clinical Counselor credentialed from the Wisconsin Board of Marriage & Family Therapy, Professional Counseling & Social Work obtained prior to hire date or job transfer date.

• Counselor credentialed from the Wisconsin Board of Marriage & Family Therapy, Professional Counseling & Social Work obtained prior to hire date or job transfer date.

• Social Worker specializing in Clinical Practice credentialed from the Wisconsin Board of Marriage & Family Therapy, Professional Counseling & Social Work obtained prior to hire date or job transfer date.

• Social Worker credentialed from the Wisconsin Board of Marriage & Family Therapy, Professional Counseling & Social Work obtained prior to hire date or job transfer date.

• Therapist specializing in Marriage & Family credentialed from the Wisconsin Board of Marriage & Family Therapy, Professional Counseling & Social Work obtained prior to hire date or job transfer date.

• BLS Provider preferred. American Heart Association or American Red Cross accepted.

• Supervisor credentialed from the Wisconsin Department of Safety and Professional Services preferred.

Education:

• High School diploma equivalency with 2 years of cumulative experience OR Associate''s degree/Technical degree OR 4 years of applicable cumulative job specific experience required.
