Education:Master'sDegree in SocialWorkRequired

License(s)/Certification(s)Required:LicensedClinicalSocialWorker - LCSW

Experience:

1. Threeyears' experience in a health/mentalhealthsetting as a LicensedClinicalSocialWorker.
2. Twoyears' experienceworking in an under-servedcommunity.

FUNCTIONSOFTHE JOB:

Essential Functions

1. Meetwithmedicalteam each morning for dailyhuddle
2. Moveabouttheclinicthroughout the day to seepatients in examrooms in the integrated,warmhand-offprocess, in collaborationwithmedicalproviders.
3. Providebehavioralinterventionsforpatientswithchronicillnesses to improve their healthstatus.
4. Providebehavioralassessments and interventionsforpatientswhohavepositivescores on depression,substanceuse, and otherscreeningtoolsused in the health center.
5. Providebriefassessmentsthatcombineknowledge of DSM V criteria,impact of medicalconditions,culturalconsiderations, and systemicbarriers.
6. Providecrisisinterventionincludingriskassessmentandappropriatefollow - up planning to patientsreferred by their PCP.
7. Addresssocialandemotionalfactorsrelated to the patients'medicalconditions.
8. Collaboratewithpatient to develophealthself-managementgoals.
9. Providefeedback to medicalprovider.
10. Completedocumentationof patientencounterswithin48hours.
11. Meetmonthlyproductivityexpectations.
12. Performduties that are requiredforthePCHH and othergrants as needed.
13. Coordinatetreatmentwith PCP and psychiatristasappropriate.
14. Screenpatientsforappropriatepsychiatryreferral.
15. Provideconsultation and serve as a resource to otherAffiniaHealthcareprograms.
16. Leadorco-leadclasses and patientgroups as needed to improvepatienthealthstatus.
