FUNCTIONSOFTHE JOB:
Essential Functions
Meetwithmedicalteam each morning for dailyhuddle
Moveabouttheclinicthroughout the day to seepatients in examrooms in the integrated,warmhand-offprocess, in collaborationwithmedicalproviders.
Providebehavioralinterventionsforpatientswithchronicillnesses to improve their healthstatus.
Providebehavioralassessments and interventionsforpatientswhohavepositivescores on depression,substanceuse, and otherscreeningtoolsused in the health center.
Providebriefassessmentsthatcombineknowledge of DSM V criteria,impact of medicalconditions,culturalconsiderations, and systemicbarriers.
Providecrisisinterventionincludingriskassessmentandappropriatefollow - up planning to patientsreferred by their PCP.
Addresssocialandemotionalfactorsrelated to the patients'medicalconditions.
Collaboratewithpatient to develophealthself-managementgoals.
