The Behavioral Health Consultantprovidesa range of psychological screening, assessment, diagnostic, and brief therapeutic interventions for patientsreferredfor behavioral health treatment. TheBehavioral Health Consultantworks in a primary care medical setting and serves to integrate physical health with behavioral healthutilizingthePatient-Centered MedicalHomemodelof care.

ESSENTIAL DUTIES AND RESPONSIBILITIES
Clinical Care
Provides on-site behavioral health screenings, assessments, consultations, andreferralfor patients in a primary care medical setting.
Provides brief and short-term behavioral intervention and crisis intervention as needed
Implementsclinical pathways and protocols for treatment of selected chronic conditions.
Documentsappropriate healthrecords in the electronic medical record.
Follows up todeterminereliability and validity of treatment used; makes recommendationsregardingmodification to services and service delivery.
Works collaboratively with Health Center staff and local human service organizations, treatment centers, and other community-based organizations tofacilitateappropriate carecoordination, information, and referrals.
Works collaboratively withmembers of their clinical team and theBehavioral Health Department staff to support patient engagement following referral from primary care.
Program and Quality Management
Participates in the review of clinical issues andbehavioral healthprogram policies and procedures.
Participates in quality improvement projects as requested, including researching client data/trends,utilizingdata measures to prioritize client contact, andassistingin streamlining internal procedures.
Completes quarterly peer reviews and assessment of chart documentation and adherence.
MINIMUM REQUIREMENTS
Master's degree in social work,psychologyor counseling degree
Licensed in the State of Missouri as aPh.D., LPC or LCSW
Sixmonths ofdirectly relatedexperience which may include internship and/or practicum
PREFERRED REQUIREMENTS
Experienceworking with clients who have chronic health conditions,and/orsubstance abuseconditions
Experience in providing both group and individual therapy

Working Conditions
General working conditions are in ahealthcarefacility. As such, there isa potentialexposure tovirus,disease, andinfectionfrom patients while performing the duties of this job.May experience traumatic situations includingpsychiatric traumatized and deceased patients. Travel may be expected to community meetings, client homes, or other agencies.
