What You’ll Do & Bring
As a Behavioral Health Case Manager, you'll play a critical role in connecting patients with the resources, services, and support they need to achieve improved health outcomes and overall well-being.
Key Responsibilities:
- Conduct assessments, care planning, and ongoing case management services
- Coordinate health and human services to meet individual patient needs
- Utilize evidence-based assessment tools such as CANS and ANSA
- Monitor patient progress and support treatment planning
- Serve as a liaison between patients, providers, and community resources
- Support quality initiatives aligned with CCBHC, HEDIS, ACO, and other programs
- Assist with care coordination and patient advocacy efforts
- Provide support in Behavioral Health Assessment Center operations as needed

- Knowledge of crisis intervention principles and behavioral health best practices
- Strong advocacy skills and attention to detail
Required Qualifications:
- Ability to obtain Texas HHS Community Health Worker (CHW) certification within 12 months of hire
- Ability to complete the Texas Community Partner Program within 12 months of hire
- Ability to obtain QMHP-CS certification within 2-3 months of hire
- Bachelor's degree in a qualifying behavioral health-related field, Registered Nurse (RN), or Licensed Practitioner of the Healing Arts (LPHA)
Preferred:
- Bilingual in Spanish
- Background in social work
- Experience with risk assessments, crisis management, and de-escalation techniques
- Leadership experience in volunteer or professional settings
