    Job Description: Are you a compassionate and driven individual with a knack for problem-solving and a heart for helping others? Join our team as a Per Diem Behavioral Health Case Manager! As a crucial member of our dynamic team, you'll be at the forefront of empowering individuals and families to overcome obstacles and achieve their full potential.

    Responsibilities:

    • Conduct comprehensive assessments to identify the unique needs and strengths of clients and their families.
    • Develop individualized treatment plans in collaboration with clients, families, and interdisciplinary team members.
    • Provide ongoing case management services, including advocacy, coordination of care, and referrals to community resources.
    • Monitor client progress and adjust treatment plans as needed to ensure optimal outcomes.
    • Serve as a supportive and empowering resource for clients and their families, promoting resilience and self-advocacy.
    • Collaborate closely with other healthcare professionals, including psychiatrists, therapists, and social workers, to provide holistic care.

    Qualifications:

    • Bachelor's or Master's degree in Social Work, Counseling, Psychology, or related field.
    • Previous experience in case management or behavioral health preferred.
    • Strong communication, organizational, and problem-solving skills.
    • Ability to work independently and as part of a multidisciplinary team.
