Job Title: Case Manager

Location: Glendale, AZ /Phoenix Area

About the Role

Our client is seeking a dedicated Case Manager to support individuals and families through coordinated, person-centered care. This role focuses on connecting clients to community resources, supporting treatment goals, and collaborating with interdisciplinary teams to improve long-term outcomes. Key Responsibilities

  • Provide comprehensive case management services, including assessments, service planning, referrals, and follow-up

  • Coordinate care with internal clinical teams, community partners, and external service providers

  • Advocate for clients to ensure access to appropriate medical, behavioral health, housing, and social services

  • Maintain accurate and timely documentation in accordance with regulatory and organizational standards

  • Monitor client progress toward individualized goals and adjust service plans as needed

  • Participate in team meetings, case conferences, and ongoing training

  • Ensure services are delivered in a culturally responsive, client-centered manner

Qualifications

  • Degree in Social Work, Behavioral Health, Human Services, or a related field required

  • Prior experience in case management, behavioral health, social services, or community-based programs

  • Knowledge of community resources, care coordination, and service delivery systems

  • Strong communication, organizational, and time-management skills

  • Ability to manage a caseload and work effectively in a fast-paced environment

  • Valid drivers license and ability to travel locally, if required by program

  • MUST HAVE SERIOUS MENTAL ILLNESS BEHAVIORAL EXPERIENCE
