Job Title: Case Manager

About the Role
Our client is seeking a dedicated Case Manager to support individuals and families through coordinated, person-centered care. This role focuses on connecting clients to community resources, supporting treatment goals, and collaborating with interdisciplinary teams to improve long-term outcomes. Key Responsibilities
Provide comprehensive case management services, including assessments, service planning, referrals, and follow-up
Coordinate care with internal clinical teams, community partners, and external service providers
Advocate for clients to ensure access to appropriate medical, behavioral health, housing, and social services
Maintain accurate and timely documentation in accordance with regulatory and organizational standards
Monitor client progress toward individualized goals and adjust service plans as needed
Participate in team meetings, case conferences, and ongoing training
Ensure services are delivered in a culturally responsive, client-centered manner
Qualifications
Degree in Social Work, Behavioral Health, Human Services, or a related field required
Prior experience in case management, behavioral health, social services, or community-based programs
Knowledge of community resources, care coordination, and service delivery systems
Strong communication, organizational, and time-management skills
Ability to manage a caseload and work effectively in a fast-paced environment
Valid drivers license and ability to travel locally, if required by program
MUST HAVE SERIOUS MENTAL ILLNESS BEHAVIORAL EXPERIENCE

Pay Details: $18.00 to $23.00 per hour
