Join Heritage Health Network as a licensed Behavioral Health Care Manager (BHCM)<\/b> and support high -need Medi -Cal members through our Enhanced Care Management (ECM) program.
<\/p>

This hybrid role includes 2–3 days in the field and 2–3 days in the office, allowing for flexibility while maintaining meaningful, in -person member engagement.

What You’ll Do<\/b><\/span>
<\/h3>

• Serve as the primary point of contact for members with behavioral health and psychosocial needs
  <\/li>
• Conduct comprehensive assessments and develop person -centered care plans
  <\/li>
• Coordinate care across behavioral health providers, medical teams, and community resources
  <\/li>
• Conduct in -person home and community visits
  <\/li>
• Support transitions of care and post -discharge follow -up
  <\/li>
• Provide education, coaching, and trauma -informed support
  <\/li>
• Maintain accurate documentation and meet program requirements
  <\/li><\/ul><\/div><\/span>
  

  Requirements<\/h3>

  • Education:<\/b> Master’s degree in Social Work or Psychology required
    <\/span><\/li>
  • Licensure:<\/b> Active Licensed Clinical Social Worker<\/span><\/span>, Licensed Marriage and Family Therapist<\/span><\/span>, or Licensed Professional Clinical Counselor<\/span><\/span> required
    <\/li>
  • Experience:<\/b> 1+ years in care management, behavioral health, or related setting (Medi -Cal/ECM preferred)
    <\/li>
  • Bilingual English/Spanish preferred
    <\/li>
  • Strong communication and care coordination skills
    <\/li>
  • Knowledge of behavioral health, SDOH, and community resources
    <\/li>
  • Comfortable with field work and home visits
    <\/li>
  • Valid driver’s license, insurance, and reliable vehicle
    <\/li><\/ul><\/div><\/span>
