    As a WCBH Care Coordinator, you'll work closely with our Clinical Team to provide care coordination and case management for a caseload of clients receiving community-based and in-office skills-based therapy. You'll assess client needs, coordinate care, and connect individuals with community resources, while supporting clinical processes associated with mental health treatment.

    Essential Duties and Responsibilities:

    • Help clients and families understand treatment options and make informed decisions through regular in-office and phone check-ins
    • Work with the treatment team to facilitate coordination of care and information sharing
    • Participate in developing and implementing treatment plans, case management, and other clinical documentation
    • Promote effective use of clinical resources, community resources, and other social supports
    • Assist clients in accessing resources, including sharing information and applying for benefits
    • Coordinate and communicate internally and externally to ensure continuity of services

    Education:

    • Bachelor's degree in Psychology, Social Work, Education, Rehabilitation, Human Services or related field is required with 2 years of experience preferred

    Experience:

    • 2 years' related experience

    Required Skills:

    • Excellent communication, problem-solving, time management, and organizational skills required.
    • Must be flexible and demonstrate the ability to work with minimal supervision.
    • Excellent computer skills and knowledge of Microsoft Office
    • Ability to learn necessary computer and technical skills essential to the position and Agency needs.
    • Ability to relate to staff, clients and community around a mental health system.
    • Maintain skill competencies in customer service, quality improvement, compliance and safety.

    Preferred Skills:

    • Knowledge of and experience with electronic medical record system, preferred.
    • Experience providing targeted case management services.
    • Prior experience in a community mental health environment.
    • Knowledge of evidence-based mental health treatments such as Dialectic Behavioral Therapy (DBT), Cognitive Behavioral Therapy (CBT), Enhanced Illness Management and Recovery, and Prolonged Exposure for PTSD.

    Physical Requirements:

    • Ability to access all Agency sites

    Other:

    • Ability to travel in connection with the job duties assigned
    • Have and maintain a valid state driver's license
    • Have reliable transportation
    • Have and maintain personal automobile insurance
    • Ability to obtain and maintain Agency and/or State of New Hampshire required background check(s)
    • Understanding of the importance of confidentiality and the ability to maintain that confidence.
