Beahvioral Health Therapist (LPCC, LISW)

Licensure and Credentialing: Must be credentialed and in good standing in a behavioral health discipline, including but not limited to: Social Worker Trainee (SW-T) Marriage and Family Therapist (MFT) Licensed Independent Social Worker (LISW) Licensed Professional Clinical Counselor (LPCC) Must possess or be eligible to obtain: National Provider Identifier (NPI) Ohio Medicaid Provider ID Successful completion of background checks and credentialing verification is required prior to contract start. Collaborate with therapists, case managers, CHW supervisors, clients, and family members to integrate behavioral health services with long-term services and supports (LTSS) and other community-based resources.

The Behavioral Health Therapist provides direct clinical counseling, assessment, and care coordination services as part of an integrated care team. This role is responsible for conducting comprehensive clinical assessments, developing individualized service plans, delivering evidence-based therapeutic interventions, and coordinating care across multidisciplinary partners. The Contract Therapist maintains an active caseload and ensures services are delivered in a client-centered, ethical, and compliant manner in alignment with Medicaid, state, and program requirements. Client Assessment and Care Planning Estimated Effort: 30% Complete comprehensive clinical assessments, including mandated tools such as CANS Assessments, within required timelines to determine clinical needs, mental status, substance use concerns, and eligibility for services. Utilize clinical judgment and assessment triggers to identify changes in a clients medical, behavioral, or psychosocial status and adjust care plans accordingly. Develop, implement, and update Individual Service Plans (ISPs) and Case Management Plans in collaboration with clients, caregivers, physicians, case managers, and other members of the care team. Ensure all assessment and planning activities comply with state regulations, Medicaid requirements, and organizational standards. Direct Clinical and Case Management Services Estimated Effort: 40% Provide individual counseling and crisis intervention services to clients experiencing psychological, emotional, or adjustment-related difficulties. Develop and facilitate group counseling and therapeutic programming as appropriate. Deliver services including Community Psychiatric Support Treatment (CPST) and Therapeutic Behavioral Services (TBS) in accordance with program and billing requirements. Conduct telephone, face-to-face, and home visits as required for monitoring, intervention, and support; local travel is required based on caseload and state regulations. Employ Motivational Interviewing techniques and evidence-based clinical approaches to support client engagement, skill development, and progress toward independence. Assess barriers to care and coordinate linkage to crisis or emergency services when necessary. Maintain an active caseload and conduct ongoing outreach, monitoring, and follow-up to evaluate service effectiveness. Coordination, Collaboration, and Consultation Estimated Effort: 20% Coordinate care to ensure services are safe, timely, effective, efficient, and client-centered. Collaborate with therapists, case managers, CHW supervisors, clients, and family members to integrate behavioral health services with long-term services and supports (LTSS) and other community-based resources. Participate in regular clinical supervision for case consultation, service planning, and ISP updates. Serve as a clinical resource to partner agencies by providing assessment input, consultation, and education related to behavioral and mental health needs. Provide outreach, education, and consultation to community agencies, parents, educators, and other groups to enhance capacity for supporting children, adolescents, adults, and families. Documentation, Compliance, and Professional Responsibilities Estimated Effort: 10% Maintain accurate, timely, and complete clinical documentation in approved electronic record keeping systems. Ensure confidentiality and security of Protected Health Information (PHI) in accordance with HIPAA, Medicaid, and contractual requirements. Attend and participate in required meetings, trainings, and conferences related to clinical practice, compliance, and service delivery. Perform additional clinical or administrative assignments as directed by the supervisor, including provision of outpatient services to children and adolescents when needed. Minimum Qualifications Education: Bachelors degree in Human Services or a closely related field required. Experience and Knowledge Minimum of one (1) year of experience in Behavioral Health and/or Substance Use Disorder services preferred. Working knowledge of Medicaid billing and coding requirements. Experience providing CPST and TBS services. Demonstrated knowledge of crisis intervention and de-escalation techniques. Proficiency with electronic health record and documentation systems. Working Conditions This contract position involves office-based, community-based, and home-visit services. Local travel is required based on caseload needs. The role requires managing multiple clients, maintaining regulatory compliance, and meeting documentation timelines, with moderate levels of stress associated with clinical service delivery.
