Description:The Service Coordinator reports to the Manager of Operations, ensuring quality measures by developing and monitoring implementation of quality enhancements and improving communication to enrich the quality of life for the people and families served by AHG. The House Manager will ensure quality enhancement principals by establishing and implementing quality improvement and an effective communications program that assures quality standards are met to ensure people served meet their personal outcome measures. Consistent attendance and ability to travel is a general qualification for this job. Responsibilities:• Ensure AHG, LLC is following policy & procedures, best practices, and standards for IDD•Makes onsite home visits to individuals served•Interview/Hire direct care staff•Supervision of direct care staff•Review assist and coordinate individuals' finances.•Develop/Monitor/Track individuals personal outcome measures and goals•Document unusual occurrences, report and follow up to operations teammembers.• Review hard copy and electronic files to ensure information is in accordancewith IDD service standards.•Training and staff development onsite as needed•Assist with development and coordination of staff schedules•Transfer Data to complete monthly reports•Coordination with team to ensure services are administered according to IDDstandards and best practices.•Monitoring of Medication Administration, audit MAR•Audits medical records and ISP data for compliance•Assists and completes Admission records on new individuals•Ensures AHG maintains compliance with service standards for DHS, IDD, CQLand PASSE regulations or requirements.Oversight of programmingo Ensure delivery of person-centered services and dignity of choiceo Complete Provider Reviews during Monthly Home visits.o Conduct area Team Meetings as neededo Review all documentation from sites to ensure compliance• And all Duties as assignedQualifications:Education: High School Diploma or GEDSkills: Demonstrated leadership• Possess excellent oral and written communicationsskills, with strong interpersonal skills.• Organizational skills• The ability to initiate and maintain successful collaborationwith staff.• 3 year experience with IDD population.Knowledge• Ability to apply sound and mature decisions• Computer experience: Word, Excel, additional software• Familiarity with healthcare laws, regulations, AHG system and IDDAdditional RequirementsValid driver's license / Acceptable driving recordDependable Vehicle and proof of Auto InsuranceTravel at least 30 % of time and must be flexiblePass State required criminal background checks and maltreatment checks
