PositionSummary:TheCareCoordinatorisresponsibleforfacilitatingtheWraparoundprocess for youth and young adults in or previously in foster care that are eligible to receive services and support through the youth services department. TheCare Coordinatorcoordinatesand schedulesFamilyTeamConferencestopromotepermanencyandprioritizesurgentrequests. The Care Coordinator maintainsinformation regarding theenvironmentofcurrentservicestoensureaflexible, responsive, culturally competent service delivery system. This position is a 2-year Federal grant funded position.

 

Technical/FunctionalExpectations:

 

1.   FamilyTeamConferences-EssentialFunction:CoordinateandfacilitateFamilyTeamConferencesinorder toachieveconsensusamongfamilyand otherparticipantsindevelopinganappropriateplanbasedon the needsofthefamiliesandtoensurecompliancewithFPOCFprocedures,values,and standards.

2.   ServiceandTreatmentDelivery-EssentialFunction:Ensureservicesandtreatmentaredeliveredfroma

strengthbasedframeworkandcustomizedinordertomeetindividualizedneedsofconsumersandare

culturallycompetent.

3.   UtilizationManagement--EssentialFunction:Authorizationandmonitoringofresources,includingagency

and community, to meetclient’sneeds.

4.   RelationshipMaintenance--Essential Function:  Foster and maintain ongoing relationships with family

members,caremanagers,providers,andidentifiedresourcesinordertocallonotherstoimplement

clientplans.

5.  AdheretoSOC Values andPrinciples-Essential Function:  This is required in orderto embraceand promulgatethevaluesandprinciplesoftheSystemofCareonaconsistentbasiswitheachfamilyand

interaction.

6.  Planning/Scheduling/Meetingmultipledeadlines:Planandschedulemeetings,conferences,and travel schedules inordertomaintainstaff schedules.

BusinessExperience:

1.   Minimumof2yearsknowledgeofchildwelfaresystems:Thisisrequiredinordertounderstandand successfullyfunction withinoursystemofcareandtoadheretoFloridastatuteand code.

2.   Strong computer/data entry skills: Thisexperienceis required in order tomaintain our utilization

managementsystem.

3.   3+ years’ Experienceworking withfamiliesandchildren: Thisexperience is essential in order to

effectivelyworkwith children and familieswithinoursystemofcare.

4.   Collaboration,negotiationandproblem-solvingskills:Thisexperienceisessentialinordertoeffectively

facilitatetheWraparound FamilyTeamconferencingprocess.

5.   ValidDriver's License&AppropriateInsurance-Essential:This position requiresavalid driver’slicensein

orderto travelthroughouttheCountyand requires$100,000/$300,000bodilyinjuryinsurancecoverage

(per FLStatute)inordertotransportclients.

 

EducationalandExperienceRequirements:

1.       A high school diploma or GED equivalent (official transcripts required) AND four or more years of full-time social work or human services experience.

  1. An associate degree from an accredited college or 60+ college credits from an accredited college AND two years of full-time social work or human services experience or two years of professional work experience.  Examples of work experience could be, but are not limited to:
    • Guardian Ad Litem or similar child advocate role
    • Family Support Worker
    • Group Home Staff
    • Teacher’s assistant/aide
    • Childcare provider/staff
    • Therapeutic Assistant
    • Behavioral health technician
    • Family intervention specialist
    • Home health aide
    • Nurse (LPN or RN) or similar profession
    • Nursing facility assistance
    • Emergency Medical Services (EMS
    • Other professional jobs that require an assessment of factors that can contribute to trauma or protective capacities with children and families.
  2. Bachelor’s Degree,Human ServicesorSocialWorkpreferred. (Related degrees are Criminology, Psychology, Sociology, Counselling, Special Education, Education, Human Development, Child Development, Family Development, Marriage and Family Therapy although other degrees may apply.)
  3. 1to2yearsrelatedexperiencerequired.
  4. 2to5yearsWraparoundandrelatedexperiencerequired.
  5. You must obtain a Child Welfare Case Manager Certification within 12 months of completion of paid pre-service training.
