    About the Role

    We are seeking a compassionate and motivated Certified Occupational Therapy Assistant (COTA) to join our rehabilitation team in a Skilled Nursing Facility setting. The COTA will work under the supervision of a licensed Occupational Therapist to provide high-quality therapy services to short-term rehab and long-term care residents.

    Our team is dedicated to helping residents regain independence, improve functional mobility, and enhance overall quality of life.

    Responsibilities

    • Implement individualized treatment plans developed by the Occupational Therapist

    • Provide skilled therapy interventions focused on ADLs, functional mobility, strength, balance, and safety awareness

    • Work with patients recovering from orthopedic surgeries, stroke, neurological conditions, cardiac events, and general deconditioning

    • Accurately document daily treatment notes and progress reports in compliance with Medicare and facility guidelines

    • Collaborate with nursing staff, PT, SLP, and interdisciplinary team members

    • Educate residents and caregivers on adaptive techniques and home programs

    • Maintain productivity standards and quality care expectations

    Qualifications

    • Associate degree from an accredited Occupational Therapy Assistant program

    • Current state license as a COTA (or eligibility)

    • Active NBCOT certification required

    • Experience in SNF or geriatric setting preferred

    • Knowledge of Medicare Part A & B documentation (preferred)

    • Strong communication and time management skills
