The Assistant Manager Clinical Services (Hospice) is a full-time position with Atlantic Visiting Nurse's (AVN), supporting the Hospice Department and provides executive support to the Hospice Director and Hospice Medical Director.
Qualifications/Experience:
Duties & Responsibilities:
Provides oversight to the Hospice Clerical Staff.
Orients and trains clerical staff
Organizes and delegates clerical team functions
Holds clerical team accountable to designated tasks
Supervises day-to-day functions.
Performance manages and completes annual performance evaluations.
Supports the Operations of the Hospice Department:
Maintains & communicates Hospice Daily Census reports/data.
Referral tracking
Admission tracking -
Communicating to disciplines
Entering NOE information
Death/discharge/Revocation tracking- communicating to team,
Assists with Consent & DNR process.
Utilizes DocuSign to obtain signatures.
Uploads in On-Base
Manages/tracks consents.
Tracks/follow-up DNRs
Coordinates services for hospice patients as indicated.
Transportation
Transfer to home care
Assists in onboarding of new staff.
Enter clinicians into Dragonfly database.
Coordinates schedules as needed.
Assists in the preparation of IDT meetings on a weekly basis.
Coordinates agendas
Coordinates attendees and schedules
Communicate Hospice patient information including admission and imminent death information.
Billing/finance
Interdisciplinary team members
Answering Service
Coordinates booking of meeting rooms and sending of invites.
Collaborates & follows up with billing & CAC department on resolutions of errors.
Registration errors
Incorrect death dates
Medicaid status
Generates, reviews, and follows- up on required reports.
Mileage reports
Authorization reports
HOPE discharge report.
Additional reports as required.
Completes end of month reporting inclusive of
CAHPS report
Dragonfly & At Home Medical reports
Accruals
Room & Board
Additional data as needed.
Maintains forms/educational materials.
Tracks inventory.
Creation and formatting of new materials
Utilizes the AHS ordering system to obtain materials from print shop.
Assists/supports Hospice Physicians
Time sheets
F2F scheduling
Coverage schedule/calendar
Oversees Bereavement Coordinator support
Coordinates/completes the end of day follow-up on prescriptions to Hospice MDs
Ensure signatures and receipts from Dragonfly for delivery.
Coordinates/communicates statuses with Care XM (answering service)
Daily calendars/schedules
Imminent death information
Weekend On-Call schedule
Participate in agency survey preparation and support.
Collaborate with Hospice Scheduler, Asst. Mgrs., and clinicians in the scheduling of visits adhering to regulatory requirements.
Contract coordination
Liaison for Medicaid Room & Board contracts for Nursing Homes & other Hospice related contracts
Liaison for Respite contracts for facilities
Administrative duties to assist & support Executive Director Home Care & Hospice, Hospice Director, and Hospice Medical Director & Hospice Leadership team:
Maintain calendars Medical Director, Hospice MD, On-Call, Time off.
Book rooms and coordinates meetings for all Hospice leadership including room set up.
Arranges for refreshments if required.
Maintains meeting minutes for all Manager/Leadership level meetings.
Assists & coordinates agency contract preparation and maintenance.
Maintains and updates corporate records as needed.
Submit reimbursements.
Other Duties as Assigned.
