Assistant Director of Nursing

Key Responsibilities:

• Assist the DON in planning, organizing, and directing all clinical services for home health patients.
• Supervise and support RNs, LPNs, and home health aides in the delivery of patient care.
• Participate in hiring, training, performance evaluations, and scheduling of clinical staff.
• Ensure compliance with regulatory requirements, accreditation standards (e.g., Medicare/Medicaid, Joint Commission), and agency policies.
• Conduct case conferences and clinical record reviews to ensure quality care and accurate documentation.
• Monitor patient outcomes and implement improvement plans as necessary.
• Participate in on-call rotation and provide clinical support during emergencies.
• Serve as a liaison between clinical staff, patients, families, physicians, and community partners.
• Help develop and implement staff education programs and competency assessments.
• Maintain accurate documentation and assist with audits, surveys, and quality assurance activities.

Qualifications:

• Current and active RN license in California.
• Bachelor of Science in Nursing (BSN) preferred.
• Minimum of 2 years of nursing experience, with at least 1 year in home health or a supervisory role.
• Knowledge of Medicare Conditions of Participation and OASIS documentation.
• Strong leadership, communication, and organizational skills.
• Ability to multitask and work in a fast-paced environment.
• Proficient in electronic health records (EHR) systems.

Work Environment:

• Office and field-based role; may require occasional travel to patient homes.
• Flexible scheduling may be required, including evenings or weekends during on-call duties.
