This role offers a unique opportunity to contribute to the development of trauma-informed, youth-centered care practices.
Key Responsibilities
- Provide clinical supervision and mentorship to Transitional Coordinators and clinicians, including those working under limited permits.
- Oversee the Transitional Coordination Team, supporting youth as they prepare to transition out of residential treatment.
- Ensure comprehensive discharge planning for youth returning home or moving into independent living environments.
- Collaborate with residential, clinical, and community providers to support continuity of care.
- Maintain a small clinical caseload and participate in treatment planning and case review.
- Support implementation of trauma-informed, youth-centered care practices.
- Assist with program oversight, documentation standards, and compliance requirements.
- Participate in leadership meetings, clinical supervision, and multidisciplinary team collaboration.
Requirements
- LCSW required.
- Master’s Degree in Social Work or related human services field.
- Minimum 2 years supervisory or leadership experience in behavioral health or social services.
- Experience working within a residential treatment required.
- Experience working with youth with Serious Emotional Disturbance (SED) or adolescent behavioral health strongly preferred.
- Must be comfortable working with families and coordinating discharge planning and community resources.
