Position: ACT Case Manager

Location: Full-Time, On-Site
Salary: $$21.73 – $22.61

As a member of our Assertive Community Treatment (ACT) team, you’ll help individuals with severe and persistent mental illness manage symptoms, integrate into the community, and achieve the highest possible quality of life. This role follows the ACT fidelity model and Wellvance policies.

What We’re Looking For

• Bachelor’s degree in a human services field
• One year of experience in the mental health field
• Certification as QMHP, QIDP, CMHP, or limited/full Michigan licensure in your field
• Ability to work a flexible schedule and travel within our service area
• Valid Michigan driver’s license, vehicle insurance, and good driving record
• U.S. citizenship or legal authorization to work in the U.S.
• Participates in ES on-call rotation
