The Ambulatory Pharmacist Practitioner provides comprehensive pharmaceutical care and medication management services for patients receiving care through UF Health Jacksonville Ambulatory Pharmacy. This centralized role supports medication access, optimization, adherence, and specialty pharmacy services across multiple service lines while collaborating closely with providers, nurses, care coordinators, and pharmacy staff.
Key responsibilities include Medication Therapy Management (MTM), prior authorization support, medication access services, specialty and specialty-lite pharmacy program management, patient education, and pharmacy workflow related to prescription processing, refill management, shipping, and medication delivery operations. The pharmacist serves as a resource to patients and healthcare teams to promote safe, effective, and cost-conscious medication use while identifying and resolving medication-related problems.
The Ambulatory Pharmacist Practitioner supports the organization's integrated ambulatory care model, which includes embedded clinical pharmacy services within select outpatient clinics, by coordinating medication management and specialty pharmacy services that enhance patient access, adherence, and clinical outcomes.
The position also participates in quality improvement initiatives, accreditation and regulatory activities, and academic programs while serving as a liaison between patients, providers, payers, and pharmacy operations.
UF Health Jacksonville Ambulatory Pharmacy provides integrated ambulatory and specialty pharmacy services to a diverse patient population, including Medicare, Medicaid, commercial, employee, and charity care patients. Through our 340B program, comprehensive medication access services, and coordinated pharmacy care model, we help patients overcome barriers to therapy and achieve optimal outcomes.
Our specialty pharmacy program provides comprehensive support for patients receiving complex therapies through clinical monitoring, refill coordination, adherence management, prior authorizations, financial assistance coordination, and medication delivery services.
In collaboration with embedded clinical pharmacy services throughout the health system, our ambulatory pharmacy team supports continuity of care, medication optimization, and enhanced patient access across multiple outpatient specialties.

Required Education: Bachelor's degree in Pharmacy -OR- Pharm D. degree (Doctor of Pharmacy).
Preferred Education: Post-graduate residency training.
Necessary Skills: Attention to detail and ability to distinguish drug products. Fine motor coordination and manual dexterity. Computer skills and accuracy in calculations. Excellent communication and customer service skills. Supervision of technician employees.
Required Licensure/Certifications: Registered Pharmacist in the state of Florida -OR- Eligible for Pharmacist license in the State of Florida within 90 days of the date of hire/transfer.
Required Experience: Less than one year Pharmacist/intern experience.
Preferred Experience: Experience and/or advanced training in specific practice area.
Supervision: Supervisory responsibility of 3-18 employees.
