    Job Details:

    This position is structured as two distinct role profiles. The employee''s time will be allocated equally between both profiles, with 20 hours per week (50% effort) dedicated to each set of responsibilities.

    The Ambulatory Nurse is a nursing staff position, which reports to the Administrative Manager. Performs treatments, administers medications, assists procedures performed without anesthesia while utilizing aseptic technique, assists with employee education and orientation and provides patient/family education with the goal of improving health outcomes. Involves family/significant others in planning health care and health decisions. Delegates and evaluates health care to assistive personnel following the hospital plan for nursing. Recognizes and advocates for each patient being individual with health care needs. Provides considerate, respectful care affirming the patient's right to make decisions regarding his/her medical care.

    The Nurse Navigator collaborates with medical, nursing, social services, and other staff to ensure that a comprehensive care plan is implemented for all pediatric patients and for adult patients as appropriate within the clinical ambulatory settings. This role also provides education to patients and parents in partnership with nursing staff in the designated areas. In addition, the Nurse Navigator supports staff and community education on the management of children and young adults across multiple departments. The position requires a strong understanding of growth and development from infancy through adulthood.

    Required Qualifications

    Bachelor of Science in Nursing (BSN)

    Registered Nurse (RN) license in the state of Ohio

    1-2 years of nursing experience

    CPR certification

    Proficient computer skills

    Strong customer service and outreach skills

    Preferred Qualifications

    Master's degree in Nursing

    Proficient in Microsoft Office Suite, including Word, Excel, and PowerPoint

    Experience operating and troubleshooting audiovisual (AV) equipment

    Department Specific Job Details:

    Shift: 7:45AM - 4:30PM, Monday - Friday

    Will sometimes require traveling to other locations and conference attendance.

    The Inflammatory Bowel Disease (IBD) Nurse Navigator is an experienced RN, acting within their professional boundaries, who will support the GI Division at Dayton Children's Hospital for the GI Department in delivering high-quality care to patients living with a chronic disease, based on assessed needs, established standards of care, and according to policy and procedures. The IBD Nurse Navigator is responsible for ensuring the provision of quality patient care in the appropriate setting through care coordination, case management, and transitions of care to different levels of care. In collaboration with the Medical Director of the Inflammatory Bowel Disease Center, the family and other members of the health care team, coordinates individualized plans of care and communicates the plan to appropriate stakeholders. Ensures high-quality care is provided as efficiently and cost effectively as possible.
