    Penn Medicine is seeking a full-time, day shift Ambulatory Clinical Pharmacy Specialist for downtown Philadelphia.

    The Clinical Pharmacy Specialist - Ambulatory Care develops, implements, and coordinates advanced clinical pharmacy services across ambulatory practice settings. Working collaboratively within interdisciplinary care teams, the specialist provides comprehensive medication management for patients with acute and chronic conditions, with a focus on optimizing therapeutic outcomes, minimizing medication-related risk, and supporting evidence-based prescribing. Responsibilities include conducting detailed medication assessments; supporting individualized medication and disease-state management through collaborative care models; providing longitudinal follow-up for patients with complex or high-risk medication needs; and delivering targeted medication education to patients and providers. The role also contributes to clinic-level quality and safety initiatives and promotes the safe, effective, and cost-conscious use of medications aligned with clinical and operational priorities.

    Credentials, Education or Equivalent Experience:

    • PA Pharmacist Licensure (Required)
    • Board of Pharmaceutical Specialties Certification in area of specialty practice (or Pharmacotherapy if no specialty certification available) (Must be obtained within 1 year of hire)
    • Bachelor of Science Degree OR Doctor of Pharmacy Degree from an accredited College of Pharmacy (Required)
    • PGY1 Pharmacy Residency OR 10 years of relevant hospital experience (Required)
    • PGY2 Pharmacy Residency OR 3 years of relevant hospital experience (Preferred)
