Care Plus Hospice Care LLC is seeking a compassionate and dedicated After Hours Hospice Nurse (On-Call) to join our team in the Phoenix AZ valley. This is a meaningful opportunity to provide expert clinical care and emotional support to patients and families during their most critical moments — after hours when they need it most. Responsibilities:

• Respond to after-hours patient and family calls, providing triage, guidance, and in-person visits as needed
• Conduct skilled nursing assessments and interventions for hospice patients in home or facility settings
• Manage and adjust pain and symptom control measures in accordance with the patient's care plan
• Communicate effectively with the on-call physician and interdisciplinary team regarding patient status changes
• Provide emotional support and education to patients and families during end-of-life transitions
• Document all clinical encounters, interventions, and patient updates accurately and in a timely manner
• Coordinate after-hours care needs including medication changes, equipment, and emergency support

Requirements:

• Active RN or LPN license in the state of Arizona (RN preferred)
• Minimum 1-2 years of hospice, palliative care, or home health nursing experience
• Strong clinical assessment and critical thinking skills in autonomous settings
• Ability to work on-call shifts including evenings, weekends, and holidays
• Valid driver's license and reliable transportation for patient home visits
• Proficiency in electronic medical records (EMR) documentation
• Compassionate, calm, and professional demeanor when working with patients and grieving families
