Role OverviewThe Advanced Therapies Nurse Specialist provides comprehensive coordination of all services and systems to provide an organized and efficient multi-disciplinary team approach for the care of patients receiving advanced therapies at TGH. The role will help to shape the development and evolution of advanced therapies at TGH, serving to enhance its overall effectiveness, productivity, and quality. This position serves as the glue for facilitating an organized and comprehensive approach to the management of patients undergoing advanced therapies across settings of care and the trajectory of illness – delivering the best possible clinical outcomes, patient satisfaction,and provider satisfaction in the process. The role reports to the Advanced Therapies Clinical Operations Manager.Core ResponsibilitiesPatient & Therapy Coordination / FacilitationCoordinate the entry and integration of prospective patients into and through the process ensuring timely evaluations and collaboration among the different disciplines involved, including but not limited to the following duties:Execute bespoke pathways tailored to each product covered by the advanced therapy program across TGH service lines and therapeutic programsDevelop timeline for therapy based on patient status, product availability, and program scheduleEnsure evaluations including testing are completed in accordance with program SOPs and in the appropriate timeframe to meet timelineCoordinate with manufacturer to ensure manufacturing slot is secured and facilitated and financial assistance is facilitated and secured (if appropriate)Coordinate with insurer, pharmacy vendors and/or TGH pharmacy to ensure mobilization medications are available within desired timeframeCommunicate with patients and caregivers across care journey, including appointments, testing and procedures, consents, and documentationCollaborate with the inpatient and outpatient medical care teams and other services (apheresis, radiology, radiation therapy, cell therapy laboratory, OR, etc.) to coordinate and schedule mobilization, collection, admission and infusionCoordinate with relevant stakeholders if/when there is overlap with research (expanded access protocol, off-label use, etc.)Assess patient/family physiologic, psychosocial and financial needs. Based upon assessment, make recommendations to social work, financial planning, etc.Coordinate transportation, housing, meals, and other patient support programsEducate patients and families to assist understanding of the disease, processes and procedures related to mobilization, collection and infusion of advanced therapy productsCollaborate with program' Benefits Specialists to ensure all required approvals are obtained for procedures as appropriateCoordinate all aspects of patient's care, from enrollment to transition of care, and through all follow ups, including facilitating data collection requirementsTransition patient case to appropriate team within or outside of the hospital partnerAccountable to ensure that all patients who are being evaluated receive care in an organized, consistent, efficient, and high-quality mannerProgram DevelopmentParticipate in the development of systems and tools for management of the function,Including:Developing and operationalizing workflows for internal and external patient identification, enrollment, care coordination, and support across all products, disease programs, and sites of careEstablishing data collection/tracking standards and the development of a data repository and tracking tool over timeParticipate in the development and implementation of clinical programs and procedures and the program's performance improvement and quality programs to promote positive outcomesEnsure any data handled at any point throughout the patient journey is tracked in an accurate, organized and consistent manner and made available to quality and/or data management personnelServe as a clinical and educational consultant to the nursing staff, medical staff, management, other members of the health care team and external vendors regarding the management of advanced therapy patientsOpportunities to participate in the growth of the program and business unit within and outside TGH, including participating in/traveling to the implementation/integration of programs at other health systemsContribute to the development of centralized Nurse Specialist function, structured to augment on-site coordination at TGH and other health system partnersCredentials & RequirementsRegistered Nurse - FL license2+ years oncology, transplant, and/or advanced therapies experienceBachelor of Arts or Science
