    Position: Advanced Practice Nurse (Nurse Practitioner) - Outpatient Clinic

    Qualifications:

    • Master's degree from an accredited school/college of nursing OR master's degree from an accredited Physician Assistant Program
    • Current license as an Advanced Practice Registered Nurse in the State of Illinois
    • Certification as an Advanced Practice Registered Nurse by a national certifying body approved by the Illinois Department of Regulation APRN Board
    • Minimum of two (2) years of APP experience
    • Bilingual with Spanish, strongly preferred

    Responsibilities:

    • Monitor patients and order tests and procedures in the outpatient setting
    • Manage a wide range of health problems through clinical expertise
    • Ensure positive health outcomes for assigned patient population
    • Perform health assessments and physical examinations
    • Provide direct clinical care, including prescription and ordering of medications and therapies
    • Evaluate patients' need for further healthcare
    • Document care and education provided in medical records
    • Refer patients to other disciplines when necessary
    • Integrate research-based knowledge in decision making
    • Advocate for patients, families, coworkers, and the community
    • Maintain licensure and certifications through continuing education
    • Collaborate with physicians and healthcare professionals
    • Participate in quality improvement and research initiatives
    • Provide comprehensive teaching strategies for patients and their families
    • Develop and participate in continuing education programs for staff
