We are seeking In‑Home Assessment Providers (Nurse Practitioners or Physician Assistants) to support our health plan partners by delivering high‑quality, in‑home preventive health assessments.

In addition to this flexible fee‑based model, you'll have the opportunity to support meaningful health outcomes, improve access to care, and work independently with the full support of a dedicated clinical team.
What You Will Do - Essential Functions
As an In‑Home Assessment Provider, you will perform comprehensive health risk assessments (HRAs) for Medicare, Medicaid, and ACA members within their homes. These assessments help identify care needs, support early intervention, and improve care coordination.

Who You Are
Required Qualifications:
Active, unrestricted NP or PA license in the state where assessments will be performed.
Board certification (ANCC, AANP, or NCCPA).
New York candidates: Also accepting board-certified or board-eligible MDs.
Current BLS certification.
Valid driver's license, reliable transportation, and auto insurance.
Ability to pass all background checks-including criminal, SSN validation, education, employment verification, healthcare sanctions, sex offender registry, and drug screen.
Strong clinical assessment and diagnostic skills.
Proficiency with EMR systems and comfort using digital tools.
